TOURNAMENT FORMS

Tournament Player Verification - The 3 proofs needed to be dated February 1, 2020 - January 31, 2021 (FOR NEW PLAYERS)

School Enrollment Form  - The player needs to be enrolled in the school prior to October 1, 2020 (FOR NEW PLAYERS)

A new form is not required if player is returning and has previous form with proofs attached.  If the player changed schools or residence a new form is needed.
